Le programme COPILOT propose un accès rapide à des évaluations gériatriques multidisciplinaires pour les personnes âgées. Le but est de fournir un soin dans leur environnement de vie, réduisant ainsi les hospitalisations inutiles et améliorant les résultats pour les patients.
Responsabilités Principales
Le rôle de l’Allied Health Assistant consiste à :
- Fournir des services cliniques de haute qualité aux clients et à leurs familles, sous la supervision d’un professionnel de santé.
- Optimiser les résultats de santé des clients pour les maintenir actifs dans la communauté.
- Effectuer des tâches administratives et organisationnelles nécessaires à la bonne marche du service.
Environnement de Travail
SWSLHD (South Western Sydney Local Health District) offre une gamme complète de services communautaires visant à améliorer la santé des individus dans la région. Le service soutient des populations diverses et inclut des programmes spécialisés pour les enfants, les jeunes et les personnes âgées.
Le personnel est composé de plus de 800 professionnels de santé, notamment des travailleurs en santé aborigène, des médecins et des infirmiers, garantissant ainsi un soin de qualité à proximité des lieux de vie des patients.
Critères d’Éligibilité
Pour postuler, il est essentiel de répondre aux critères suivants :
- Certificat IV en Assistance en Santé ou qualification pertinente, ou expérience équivalente de 12 mois avec engagement à obtenir le Certificat IV dans les 12 mois suivant l’embauche.
- Capacité à prioriser les tâches et à respecter les délais sous supervision.
- Compétences en communication interpersonnelle, écrite et verbale.
- Capacités organisationnelles, autonomie et travail en équipe.
- Permis de conduire C-Class (incluant P1 et P2). Les nouveaux résidents de l’NSW doivent obtenir un permis C-Class dans les trois mois suivant l’embauche.

The Community Older Persons Intervention and Liaison Outreach Team (COPILOT) is a dynamic and exciting service that provides rapid access to multi-disciplinary geriatric assessment (medical, nursing, and allied health) for older people living in the community setting. Referral enables care to be implemented in the patient’s home environment, avoiding unnecessary transfer to hospital, and leading to improved patient outcomes.What You’ll be Doing
The Allied Health Assistant will provide a high quality clinical service to clients and their families/carers under the supervision of an Allied Health Professional to optimise health outcomes and support clients keeping well in the community. The Allied Health Assistant is also responsible for undertaking designated departmental and administrative tasks necessary to support service delivery.Where You’ll Be Working
SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live. - Certificate IV in Allied Health Assistance or relevant qualification or Minimum 12 months full-time equivalent relevant experience with commitment to undertake the Certificate IV in AHA within 12-month period.
- Demonstrated ability to prioritise workload, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required) as delegated by supervising Allied Health Professional
- Demonstrated effective interpersonal, written and verbal communication skills with the ability to communicate across all levels.
- Demonstrated good organisational skills, ability to work independently and as a member of a team.
- Applicants must possess an Australian C-Class driver’s license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner’s licenses are not permitted.
