Mission et responsabilités :
Le Clinical Nurse Specialist (CNS) pour les jeunes adultes et adolescents atteints de cancer est un poste clé au sein de CHI @ Crumlin. Ce rôle repose sur cinq concepts essentiels définis dans le Framework for the Establishment of Clinical Nurse Specialist Posts. Le CNS travaillera principalement avec des patients âgés de 13 à 24 ans, qui présentent des cas de cancer, et collaborera étroitement avec une équipe multidisciplinaire.
Les principales responsabilités incluent :
- Expertise Clinique : Fournir des soins holistiques adaptés à cette population unique, tout en collaborant avec les membres de l’équipe multi-disciplinaire pour assurer une continuité des soins.
- Transition du Soins : Participer au développement de programmes de transition de soins entre CHI et St James’s Hospital, facilitant ainsi le parcours de soins des jeunes patients.

Critères Essentiels

📝 Description du poste : Who We Are:(CHI) is leading on the clinical and operational transformation of acute paediatric healthcare and consists of hospitals at Crumlin, Temple Street and Tallaght as well as an Urgent Care Centre at Connolly hospital in Blanchardstown.Description:Purpose of the RoleDeliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008Caseload: Adolescents and Young Adults aged 13-24 (+ 364 days) years at diagnosis in CHI @ Crumlin, and 0.2 WTE in St James’s hospital, Dublin· The National framework of care for AYAs with cancer recognises the unique needs of the AYA cancer population. Approximately 200 people aged 13- 24 years are diagnosed with cancer annually in Ireland. The management of AYA patients with malignancy requires a broad range of professional skills including but not limited to communication, leadership, teaching, education, and research and counselling.The CNS will focus initially on the following patient groups: AYAs diagnosed with cancer aged 13-24 (+364 days) years. This post is co-appointed with St James’s hospital and the post holder will contribute to the development of transition programmes and care continuum of AYAs in both hospitals. The post holder will care for young people in collaboration with multidisciplinary team.· The AYA CNS will act as a key worker for AYA cancer patients in CHI @ Crumlin where there is a requirement and ensure assessment and care delivery in a holistic, developmentally appropriate manner.· Must demonstrate the ability to work autonomously, with a high level of clinical expertise in identifying and providing specialist care for AYA cancer patients.· The CNS will have the expertise to tailor care to the specific needs of the AYA population and liaise with multi-disciplinary team members, shared care teams and community teams to ensure a seamless delivery of developmentally appropriate care from prevention to transition, survivorship and end of life.· The post-holder will work closely with colleagues within the national AYA caner network, community services, educational and vocational services and charity partners providing services for AYAs.Essential Criteria:(i)Be a registered nurse/midwife on the active Register of Nurses and Midwives held by An Bord Altranais and Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be eligible to be so registeredAND(ii) Be registered in the division(s) of General Nursing with the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireannAND(iii) Have a minimum of 1 years’ post registration full time experience or an aggregate of 1 years’ full-time experience in the division of the register in which the application is being madeAND(iv) Have a minimum of 1 years’ experience or an aggregate of 1 years’ full-time experience in specialist area of acute paediatric nursingAND/OR(v) Have a minimum of 1 years’ experience or an aggregate of 1 years’ full-time experience in specialist area of cancer nursing(vi) Have successfully completed a post registration programme of study, as certified by the education provider which verifies that the applicant has achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 9 or higher award (equivalent to 60 ECTS or above) that is relevant to the specialist area of care and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard (60 ECTS or above), relevant to the specialist area of care and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008).
