Description du poste
Le poste de Supporting People On Probation Navigator consiste à travailler au sein d’une équipe de praticiens pour offrir des services personnalisés de bien-être personnel aux hommes adultes et jeunes sous supervision judiciaire. Les principales responsabilités comprennent :
- Développer des programmes sur des thèmes variés, tels que les familles, le bien-être émotionnel, et l’inclusion sociale.
- Assurer un suivi des clients- avec des évaluations de risque et des plans de soutien adaptés.
- Travailler en collaboration avec d’autres agences de santé et de bien-être.
- Participer à des sessions de groupe et des entretiens individuels.
- Évaluer les résultats obtenus et contribuer activement aux objectifs fixés par *Forward* et les commanditaires locaux.
Il est à noter que des horaires flexibles incluant éventuellement des soirées et week-ends peuvent être requis. Les candidats doivent également passer des vérifications de sécurité renforcées par le ministère de la Justice britannique.
Profil recherché
Le candidat idéal doit posséder les compétences et expériences suivantes :
- Compréhension des enjeux rencontrés par les clients dans le système judiciaire.
- Expérience significative dans des services de bien-être individuel, incluant les familles, le bien-être émotionnel et l’inclusion sociale.
- Capacité à effectuer des évaluations de risque et à concevoir des plans de soutien.

Accommodation is delivered in Wales, Dependency and Recovery in London and Humberside, Personal Wellbeing in East of England, Sussex, Surrey and as a subcontractor in Kent and CheshireThese services work with Men 18+ who are under probation supervision, supporting them to navigate through their desired outcomes in order to achieve transformational change, break the cycle of offending and achieve long-term integration into society.Our services have supported thousands of people to make positive changes and build productive lives with a job, family, friends, and a sense of community. No matter what your past, we believe anyone is capable of lasting change.Role ResponsibilitiesYou will be working within a team of practitioners and subcontractors, delivering Personal Well Being Services to support adult men and young men in the Criminal Justice system. The support is tailored to individual needs, delivering a range of programmes within the categories of Family and Significant others, Lifestyle and associates, Emotional Well Being and Social InclusionThe Health and Wellbeing Practitioner will deliver client centred support depending on the requirements of the individual client. You will be required to manage a caseload, provide appropriate advice and guidance, assess, care plan; deliver structured 1:1 and group-work sessions within the categories of the Personal Well Being Services. Transitional support from prison to the community via integrated working with other Health and Wellbeing agencies, both within the prison and in the community, is an essential element of the work to prepare service users for release.You will be responsible for contributing to the targets set by Forward and the local commissioners. One of the key targets will be measuring outcomes, so co-operative working relationships with other partner agencies is vital to this role.Flexibility will be required in this role, this may include some evening/weekend shift and with reasonable notice to travel to other projects in your cluster to deliver Health and wellbeing services, enabling continuity of care for the client group.All prison-based roles will require enhanced DBS and HMPPS security vetting. Please note this process can take up to 3-6 weeks. All offers are subject to receiving both HMPPS vetting and DBS clearances.Checks will require you to provide information on the below:
- Yourself (personal information, financial information, police information, criminal history)
- Your family (parents, parents’ partners, siblings, partner(s), children)
- Co-residents
- Associations that may cause a conflict of interest with your role or the prison service.
- Background checks across police information systems on you, your family and other associates
- Credit reference checks
- Social media and Open-Source checks (these are checks on content about you that’s publicly available on the internet to ensure there’s nothing linked to you that could undermine public trust and confidence in the prison service)
- Other government and overseas agency or police checks.
The Ideal CandidatePlease see below a list of skills and experience needed for this role.
- Knowledge of the issues facing clients in the criminal justice sector.
- Excellent experience of working in the Personal Well Being or related services such as Families, lifestyles, emotional well-being and social inclusion.
- Demonstrable experience of carrying out risk assessments, comprehensive assessments and the design and implementation of SMART care plans.
- Significant experience of carrying out psychosocial interventions including using motivational interviewing techniques in both 1:1 and therapeutic group settings.
- Knowledge of the criminal justice sector
- Knowledge of the Recovery Agenda
